This authorization and release is intended to be broad. It allows Healthium, PLLC and its owners,
clinicians, employees, contractors, vendors, affiliates, successors, assigns, service providers, business
associates, researchers, licensees, and technology partners to receive, store, copy, analyze, modify,
annotate, de-identify, combine, disclose, publish, license, commercialize, and otherwise use the photos and
related information you submit, for any lawful purpose, including computer vision and artificial intelligence
model development, model training, model evaluation, product development, research, education, marketing,
publication, quality improvement, clinical operations, and commercial use.
The information may include protected health information or other sensitive information. Photos of skin can
sometimes identify a person through tattoos, scars, location metadata, face/body context, filenames, or other
visible details. Healthium, PLLC intends to safeguard uploads, but no electronic system can be guaranteed
risk-free.
This site does not provide medical advice, diagnosis, treatment, emergency response, or a patient-provider
relationship by itself. Do not upload photos for urgent care needs.
HIPAA authorization: if Healthium, PLLC is acting as a covered entity or business associate and the uploaded
materials are protected health information, you authorize Healthium, PLLC to use and disclose that information
as described above. The information authorized includes all photos, filenames, upload metadata, written
notes, contact details, and consent records you submit through this portal. This authorization expires when
Healthium, PLLC no longer needs the information for the permitted purposes, unless you revoke it earlier in
writing. You may revoke this authorization by sending a written request to Healthium, PLLC, but revocation
will not affect uses or disclosures already made in reliance on your authorization, information already
de-identified, model training already completed, backups, legal compliance records, or uses that cannot
reasonably be undone.
To the fullest extent allowed by law, you release and hold harmless Healthium, PLLC and the parties listed
above from claims, demands, damages, losses, liabilities, costs, and expenses arising from or related to your
voluntary upload, the permitted uses of the submitted materials, publication or commercialization of outputs,
privacy risks you accepted, and any later objection to uses already made. Nothing here limits rights that
cannot legally be waived.
You confirm you are at least 18 years old and submitting your own photos, or you are legally authorized to
submit photos for the person depicted. You also confirm that you have removed anything you do not want shared
before upload.
